INTRO
For obvious reasons, the face is the most visible part of your body, and that part which represents you to the rest of the world.
Even tiny characteristics of our face, if we consider them less than ideal, can be like big impediments to our level of confidence, and self image.
Changes which our face incurs due to weight fluctuations, hormone changes, sun exposure, lifestyle, age, and gravity, all tend to signal Time's determined forward march. These 'details' can steal from our vitality, and can dullen the glint in our eyes.
Maintenance is a way to control these issues. Much in the same way we 'upkeep' other aspects of, and objects in our lives, we can maintain our appearances through small, regular efforts like proper skin care, or non-invasive aesthetic procedures, or by more comprehensive, powerful, and long-lasting committments like surgical interventions.
Either way, today we have the means to maximize our appearances, and to age more gracefully.
faceLIFT
The facelift, or Rhytidectomy, ('Rhytides' is the Latin word for 'wrinkles') is a surgical procedure designed to redistribute the soft shape-forming tissue underneath our skin back to where it used to be - thereby recreating a proud, youthful cheek, to lift the jowly fluff, and then to smooth out the wrinkles by removing excess skin and tightening the remainder.
There are many techniques for facelifting. Doctor Basile chooses and tailors the most appropriate approach to create the changes you require for the goals you have.
Doctor Basile goes to great lengths to maximize and polish your face's positive natural characteristics, and avoid an unnatural, excessive, inartisitic result.
The Facelift is commonly effected alone, but often it is combined with treatment of the Neck, or Brow, or Eyelids.
neckLIFT
The neck is its own independent structure, and aesthetic unit. Many times the neck is treated in conjunction with the face, but often requires attention before the face requires any at all.
The amount of fat beneath the jaw line, and chin, the shape and size of the salivary glands, the natural posture of the main neck-shaping muscle (Platysma), how the neck formed as a youth and what it has gone through since, are the factors which will determine what approach might be best suited to create a youthful, and firm, neck shape and profile.
The appropriate approach, and why, the benefits you can expect, and the downtime involved can all be discussed after Doctor Basile visits with you in consultation.
eyelids
The eyelids are one of the first structures of our face which demonstrate our years, our stress, our sensitivity to seasonal allergens, heredity, and poor lifestyle.
Many patients come to Doctor Basile when they realize that the tired, just-got-out-of-bed look that used to linger briefly in the morning, now lasts all day. After an eyelid lifting, they enjoy a refreshed look. Their face is powerfully yet inconspicuously improved.
The eyelid lift also enjoys multiple techniques. The proper approach will be chosen by evaulating your anatomy, and your goals.
With a smooth, brief recuperative period, this procedue is tolerated even by the busiest of patients.
Often performed alone, the eyelid lift is often combined with a brow lift or neck lift.
brows
The brows are a very dynamic feature of our face, which signal an array of expressions ranging from surprise, to frustration. Unfortunately, the overall tendency of the forces acting of the brows is, you guessed it, downward.
When the brows have migrated south, espcecially on the sides, it may cause a heaviness of the lids, or ultimately a 'sad-sack' appearance. Elegant beauty in women is often associated with high, gracefully arched brows.
Lifting the brows back into position can lighten up a tired face, and even reduce or eliminate the need for an upper eyelid lift. Lifting of the brows also reduces the workload of the forehead muscle which develops those pesky horizontal lines when it works over-time to lift a heavy brow. Finally, a brow lift smooths out the scowl area between the brows long-term.
Sometimes the brow lift can be effected with the use of BOTOX. Many times the brow lift is performed alone, and often it is done in conjunction with the eyelid lift or facelift.
nose
The nose is a very personal structure to which many patients attach deep-seeded importance. The nose can blend in with the rest of the face, or it can stick out like a sore thumb. The nose can also be a 'center-piece' and source of pride and confidence. The nose has distinct ethnic, and gender related characteristics which can be strengthened or de-emphasized.
The "Rhinoplasty" is an umbrella term covering all the various techniques to alter, restore, and improve the shape of the nose. Depending on which characteristics need addressing, the Rhinoplasty may be quick and smooth to recover from, or have a bit slower and more involved down-time.
There is no good surgeon who had a "one-size-fits-all" approach any more. A large palette of techniques- all of which are designed to create millimeters of change are used to customize the nose parameters so that it lives in harmony with the face in which it resides.
MALElift
The man is not immune to changes of the face created by weight fluctuations, sun damage, thicker skin, and stronger face muscles. Some men merely ignore these issues more effectively. Other men, more in touch with the details of their appearance, appreciate the need for a modicum of maintenance.
While women out-number men as Aesthetic Surgery patients 8:1, the number of males in the United States is steadily climbing- especially in regards to the face. (In Europe the number of men and women patients are nearly equal) Specifically eyelids, the brow, and the neck have been areas of interest in Male patients.
Many more men have recently enjoyed the benefits of the non-invasive techniques- especially BOTOX- because of the limited downtime they may have for their "be all you can be" campaign.
Men have different needs and expectations. Men require different aesthetic and technical approaches. It is important that the man seeking Aesthetic counsel find a surgeon comfortable with these specific requirements.
NONsurgical
The non-invasive products and techniques being used today were developed out of a need to have ways to help patients who came to their aesthetic goals earlier in life, and because patients continue to insist on less and less downtime.
Non-surgical approached allow us to keep the face looking young and rested for longer before we need to step up to a surgical procedure. These techniques can sometimes allow us to get similar results to surgery, which we can support indefinitely. Finally, this branch of aesthetic medicine makes our surgical procedures last longer.

the BREAST
The breast, as is the entire female form, is a complicated structure in terms of anatomy, genetics, physiology, psychology, and sociology.
The breast is burdened by all of these roles and influences and yet we expect so much of them, and wish so much for them.
In the ultimate irony, the breasts tend to deteriorate in appearance directly correlated with the amount of effort used to maintain or lose weight, and exercise. In other words, the harder you try the worse they look. That's where a good plastic surgeon comes in.
In the following pages you will be introduced to the main concepts and surgical approaches to attaining or restoring the size, shape and position of your breasts.

AUGMENTATION
Breast Enlargement, has become the most popular aesthetic surgery procedure in the United States.
Highly refined techniques and materials have resulted in predictably excellent results.
The most common motivators for breast enlargement include:
* Desire for more breast upper-pole fullness.
* Freedom from padding and overly structured bras.
* Sense of Aesthetic Balance.
* Mourning of the breast size and firmness once present during or before pregnancy.
* Loss of weight resulting in a slighter chest--because of newer, healthier habits, hormone changes, or a shifting of body shape.
Augmentation mammaplasty is accomplished by introducing a small, soft pillow -like device into the chest tissues. The pillow case is made of a thin silicone rubber. The pillow stuffing is either salt-water, or silicone gel. After choosing the implant, its position, the insertion route, and size, patients will soon enjoy the bust they have long fantasized about.
LIFT
Pregnancy, Gravity, Sun Damage, Weight Gain, Weight Loss, Menopause. ...These are the enemies of the breast.
When the shape, position, skin quality and size of the breast loses this battle, a woman can fight back and restore her self image with an arsenal of operative procedures.
Minimal Scar Lifting techniques developed in Europe and South America can perk the shape of the breast, reduce the areola size, and improve nipple position.
Full Breast Lifts can re-contour the shape and posture of the breast like no bra can.
Augmentation/Lift Combinations can take control of the shape and size from the inside and out, for maximum change.
The important thing is to tailor the procedure to the individual patient as determined by her goals, expectations and starting body characteristics.

AUGMENTATION-LIFT
Loss of breast volume is a common price paid for nursing a child.
Hormones related to middle age can also cause loss of the perky shape of youth.
The fullness can be restored with a small implant.
The breast shape can be then re-contoured around it with a variety of lifting techniques.
The combination of Breast Augmentation and Lifting is a powerful procedure to be used when we want to recreate a snug, youthful bust with a little extra drama in the form of cleavage and superior breast fullness.
REDUCTION
Breast reduction refers to the significant removal of breast tissue in an effort to lighten the weight carried by the woman's torso. This is not as simple a procedure as the breast lift, and it is not usually done for simply cosmetic reasons.
These patients often have years of suffering from pains in the neck, shoulders, back, and breasts. Removal of this weight is aimed at alleviating their symptoms.
Nowadays women are more aware of this solution, and patients are becoming younger on average, with the philosophy of "why wait?". Frequently they come to see us after their mother or daughter has had this same procedure!
When the pains are lifted, so is their spirit, and lifestyle.
Breast Reduction patients are often the most satisfied of a plastic surgeon's practice.
MALE CHEST REDUCTION
Men can also have increased growth of the so-called breast area. This occurs most frequently at puberty, and after "middle age".
Young men may suffer from hormone-directed growth of the breast tissue, and in such a case it is wise to run a few tests to confirm the cause.
A few decades later, as our metabolism and lifestyle change for the worse, men can develop extra fat deposits in the chest which may be difficult to get rid of even with moderate diet and exercise.
Either way, it is good to know that there are several ways to remove this unwanted fullness. Sometimes a limited access approach with liposculpting instruments will work. Other times slightly more comprehensive efforts may be needed to restore the flat male chest contour.
As men have become more aware of their options, a larger percentage of males have become my happy patients.
the BODY
Though more private than our face, which remains constantly visible, our torso, midsection and limbs, are also influenced by life-changes, and also give us strong positive feedback when we sport a healthy, appealing shape.
The body accumulates changes related to age, life-style abuse, pregnancy, and gravity, which we can often re-adjust with surgical intervention.
Though never a good substitute for weight control or healthy habits, an aesthetic surgical procedure may be just the thing to restore a silhouette, circumference, or proportion which will allow you to stand straighter, more comfortably, more confidently.
And that will certainly show on your face after all!
lipoSCULPTURE
Liposuction is a wonderful technique of contouring or reshaping the body by removal of fat deposits from where they do not aesthetically belong.
Liposculpting is performed by vacuuming away the fat cells from under the skin. The fullness in this part of the body is thereby reduced, and the natural beauty from underneath is allowed to appear. This technique is a very effective way of reducing limited and specific areas of the body contour in order to make them blend in better with the rest of the body.
Though it remains a very popular and effective tool, it is not an good solution for overall body fullness or substitute for weight loss.
Liposuction is often combined with Tummy Tuck or Breast Lifting.
abdominoplasty
The shape of the abdomen depends on several of the anatomic layers in that region of the body. When the only problem with the contour of the belly is an abundant amount of fat below the skin, then liposuction techniques may be all that is necessary to reshape the abdomen.
But commonly the skin of the abdomen has suffered from being stretched during pregnancy, and/or weight gains. When the skin is no longer elastic enough to shrink after liposuction, or if the skin is already loose and wrinkly, then removal of skin may be the right solution. A mini-tummy tuck will then be the right choice.
Also quite commonly, the muscles below the skin and fat may be loose or separated. This is a frequent occurrence after pregnancy. In this case a full Tummy Tuck will thin and tighten all the layers of your abdomen to achieve that flatter contour.
The incisions used here usually heal very well. Despite their subtle nature, we take great care in designing the resultant scars within the coverage area of a bathing suit or panty.
Don't throw out that bikini just yet!
lower body LIFTING
The thighs, flanks, buttocks, and abdomen can sometimes pool together and, under the effects of gravity and skin stretching, cause a southward shift in the entire lower body.
This is especially true in those individuals who have lost a large amount of weight or had multiple pregnancies.
While liposculpture may do a lot to lighten the bulk from this section of the body, it is often necessary to remove a certain amount of skin to "pull up" on these structures--- to return to a youthful shape and lower body contour. The inner and outer thighs can be ironed out, and the buttock restored to its perky self!
Certainly this is a significant surgical undertaking for both the patient and the surgeon, and may require a series of carefully planned interventions, but it is sometimes the only solution for restoring the shape in that zone between the belly-button and the knees.
arm CONTOURING
The upper arm is another common area where fat likes to rest. Liposculpting can be very effective for the heavy upper arm that keeps you from going sleeveless. Sometimes, carefully executed liposcuplting will lighten the load on the inner arm skin and allow it to recoil upwards to a more athletic shape
The skin in this area is unfortunately not very robust. It can become loose and wrinkled after weight loss, or liposuction.
An Arm Lift is used to remove the excess skin, whereby the arm contour is thinned and tightened at the same time.
after weight loss TAILORING
It has become more prevalent for patients to lose significant weight. Bariatric surgery (stomach stapling, banding, or bypass) has increased in popularity 3-5 fold in the last few years.
At the same time, more and more people have adopted habits and lifestyles that have helped them lose large amounts of weight, in order to live longer and healthier.
Either way, once the body's "suit" has been stretched to a much bigger size, it's difficult for the skin to shrink around the new-found size and shape.
Tailoring the "suit" is necessary to capitalize on the hard work it took to get the weight off.
The excess skin of the arms, sides of chest, and breasts can often be addressed at the same time. This upper body lift may even include the use of a small implant to add some oomph to the bust line. The abdomen, the inner and outer thighs, and buttocks are also sites of awkward tissue abundance.
Developing a list of the patient's priorities, and then a sequence of procedures to maximize effect, and minimize the risks and down-time is the best way to approach these ever increasing scenarios.
NON-surgical
Many patients new to the field of Aesthetic maintenance begin their efforts with the use of non-surgical techniques.
Often, especially in our younger patients, all that is required to spruce up their facial appearance is an evaluation of their skin, and the adoption of a topical skin care regimen customized for their natural characteristics and needs.
When we need to control the expressive muscles of the face, which are causing wrinkels to form, we can use BOTOX® or Dysport®.
If we need to add volume to any small or significant aesthetic unit of the face, we can use one of an array of available injectable fillers.
Easy, predicatble, safe, and effective options- with no surgery.
Sometimes a more powerful procedure might be required. In such a case, the non-surgical palette of options can supplement or enhance the surical results and certainly help them last longer.
botox
BOTOX® is a brand name for an injectable medication derived from the Clostridium Botulinum bacteria.
A protein, cultured in the laboratory, BOTOX® weakens muscular activity, and has been used for over fifteen years as a treatment of chronic and painful muscular spasms of the face and neck. It was soon discovered that the skin overlying the areas where BOTOX® was being used, aged more slowly- accumulating fewer wrinkles! Patients started requesting its application to the other side of the face, though there was no medical need, so that they would stay youthful looking on both sides...
In 2002 BOTOX received FDA clearance for its use as an Aesthetic Medicine agent, though it had been used for that purpose several years prior. Dysport® is a newer Botulinum-derived neuromuscular agent with much the same role.
When placed in the face, BOTOX® works by interrupting the communication between the nerve endings which carry the electrical signals to one of the muscles of expression. By stopping the signal, BOTOX® keeps the muscle at rest.
Why does this help?
The skin overlying these muscles generally suffers from all of the movement. Creases eventually form, wrinkles accumulate, and depressions of contour develop.
BOTOX® stops this muscle movement, allowing the skin to remain unstressed. At rest the skin stays young looking, and even fixes itself. The trick is to keep giving the skin a nice extended "vacation".
Aside from the benefits to the skin, BOTOX ® allows us to balance the position and tone of certain facial structures. For example: the tight knit brow look associated with scowling can be eliminated; eyebrow position can be lifted; and lip corners de-frowned.
BOTOX® is applied in quick, painless sessions. Its effects usually start to become visible within a week, and last an average of four months. Like any medication, its effects are dose related. Risks are few, and complications rare. Yet, it is important that before its use you are thoroughly evaluated by a professional with extensive experience.
BOTOX® can be used in one specific area of the face, or multiple areas simultaneously. BOTOX® is often used concurrently with Injectable Fillers.

BOTOX is also now used to control underarm sweating. That's right!
Axillary Hyperhidrosis is a well defined problem that afflicts many people out there. When over-the-counter antipersperants aren't enough to control excessive underarm sweating, BOTOX can be applied in the underarm. It turns out that sweat is produced and expelled b tiny glads in the skin of the underarm. These glands are controlled by even tinier muscle fibers - and they are controlled by nerve impulses like any other muscle. That's where BOTOX comes in! It blocks the nerves, whic blocks the muscle activity, therby stoppoing wsweat flow!. No more embarrassing stains, and awkard situations. Injectable fillers include a variety of soft, inert, natural materials which can be injected underneath the skin in order to restore volume and strength to specific facial sites.
Fillers allow the raising of deep creases, smoothing of fine wrinkles, filling of depressed facial zones, and add volume to larger features. For example:
Laugh Lines---nasolabial lines- (creases from nose to lip corners)
Frown Lines---mesolabial lines- (creases emanating down from lip corners)
Scowl Lines---glabellar lines (grooves between the eyebrows)
Lip Lines---perioral rhytides ( lines aorund the lips)
Chin crease (supra-mental furrow)

The most popular fillers currently are the hyaluronic acid gels. HA fillers allow immediate treatment after evaluation, since hyaluronic acid is a natural skin component, and HA fillers are developed in a non-allergenic form.
